ALCOHOL AND DRUG ASSOCIATE COUNSELOR
The main purpose of this position is to provide interim/brief counseling and case management services to clients with substance use disorders with moderately complex problems as defined by standard treatment criteria. This position coordinates all treatment activities with services provided to the client by other resources and evaluates client progress in implementing the treatment plan and makes appropriate changes to ensure progress as outlined in the SATS contract Scope of Work. Incumbent will be on site at the local DHS office to assist in the identification of Temporary Cash Assistance, applicants and recipients and certain SNAP applicants and recipients along with TDAP, and Child Welfare Clients, and the referral of these individuals for assessment/testing to the appropriate treatment provider. The incumbent will act as a liaison between the FIA case manager, the MCO, if applicable and the substance abuse treatment provider. This position also provides information and input into the recipient’s status for initial and continued eligibility for benefits as outline the int contract deliverables.

Early Care Programs Community Health Nurse Supervisor
The primary role of this position is to provide ongoing clinical supervision for Healthy Families Family Support Specialists, Family Assessment Worker (CHN), and CHNs working with Healthy Families whose primary role is to provide intensive nurse home visiting to pregnant women and children up to five years of age. The goals of Healthy Families/Early Care Programs are to improve birth outcomes, strengthen families, cultivate nurturing parent child relationships, promote child health and developmental outcomes, promote family stability, and build foundations for strong family functioning. The position will proved support and supervision to staff working with families to promote family and child health through education, health promotion; and assessment and referral in collaboration with other health professionals and community agencies to support family and children’s optimal health, nutrition, safety and development. This position may carry a pro-rated caseload of families and conduct program outreach/assessments. Job duties are directed toward assuring quality of service provision with home visiting services to include ongoing nursing assessment, nursing diagnosis, and development of a plan of care with families considering their unique strengths and needs. Reflective supervision sessions will be provided to staff from the Early Care Program Community Health Nurse Supervisor to promote continued growth with the program. This position will require Healthy Families America training and general supervision is received from the Early Care Program Director.
