Early Care Programs Home Visitor
Full-Time Contractual COMMUNITY HEALTH NURSE II

The main purpose of this full-time contractual position is to assist the Garrett County Health Department in the mission of promoting, protecting, and improving the health of the county citizens. The position provides in-home support services to families and other non-nursing support staff through intensive nurse home visiting to pregnant women and children up to five years of age under the Healthy Families America home visiting program(s) to improve birth outcomes, strengthen families, cultivate nurturing parent child relationships, promote child health and developmental outcomes, promote family stability, build foundations for strong family functioning, and promote positive parenting practices. Services enable individuals to maintain independence and well-being, assist in preserving the family structure, promote functional home environments, and teach clients how to access and utilize available community resources. The position, with guidance from the Community Health Nurse Supervisor, works to improve parenting skills, reduce family stress, and meet the ongoing needs of the family unit to achieve optimal functioning. The position will promote family and child health through nursing assessment and planning, education, and linkage to other resources. The position will collaborate with other health professionals and community agencies to support families and children’s optimal health, nutrition, safety, and development. The position also provides support to paraprofessionals working with families. The Community Health Nurse II takes into account each family’s unique strengths and needs. This position will be provided with weekly reflective supervision sessions from the Early Care Programs Community Health Nurse Supervisor to promote continued growth with the program. Duties of this position also include providing childbirth education to expectant parents in accordance with Prepared Childbirth Educators Certification.
