Medical Social Worker
Our Medical Social Work services include:
- Emotional support and counseling to help cope with illness, hospitalization, or life changes
- Assistance with discharge planning and coordination of ongoing care
- Advocacy for patient rights and access to services
- Guidance in managing financial concerns related to healthcare
- Connecting patients and families to community resources such as transportation, meal programs, support groups, and housing assistance
