Maternal and Child Health Survey for Community Members

The Maryland Department of Health would like to hear from parents, caregivers, and concerned community members about their opinions on their community’s needs, concerns, and strengths. Community means the neighborhood where you live. As parents, caregivers, and concerned community members, you are your community’s health and safety experts. This information will help determine the priorities the department sets for the next 5 years on improving the health and safety of women, infants, children, teens, and families. We ask that you complete this short survey to help us better understand what is happening in your community. This survey is completely voluntary and anonymous.
