Maryland Family Planning Programs Survey - Statewide Survey - Chance to Win $50 Gift Card!

In partnership with the Maryland Department of Health and Johns Hopkins Center for Communication Programs, the Baltimore City Health Department is conducting this survey to learn about what quality reproductive health care look like for folks who are connected to the state’s network of clinics and/or have never visited our clinics but may be interested in making an appointment. They are exploring challenges with obtaining reproductive health services, educating clients of the continual need for family planning services, and raising awareness of the availability of services.

Survey Info:

Must be a Maryland resident between the ages of 18 to 45 years old. Anyone who completed the survey can enter a raffle at the end of the survey to win a $50 Amazon gift card.

Please note that this survey is different than the clinical services survey administered by the Garrett County Health Department for evaluation of local programs and services (https://garretthealth.org/new-garrett-county-family-planning-survey/).
